    The major predictors of testing positive for COVID-19 among symptomatic hospitalized patients

    The major predictors of testing positive for COVID-19 among symptomatic hospitalized patients Samson Barasa,1 Amy Ballard,1 Josephine Kiage-Mokaya, 1 Michael Friedlander,1 Geraldine Luna,2 1PeaceHealth Sacred Heart 2University of Illinois at Chicago Introduction: Increasing corona virus disease MESHD 2019 (COVID-19) pre-test probability can minimize testing patients who are less likely to have COVID-19 and therefore reducing personal protective equipment and COVID-19 testing kit use. The aim of this study was to identify patients who were likely to test positive for COVID-19 among symptomatic patients suspected of having COVID-19 during hospitalization by comparing COVID-19 positive and negative patients. Method: We conducted a retrospective chart review of patients who were [≥]18 years old and underwent COVID-19 Polymerase chain reaction test because they presented with symptoms thought to be due to COVID-19. A Poisson regression analysis was conducted after clinical presentation, demographic, medical co-morbidities, laboratory and chest image data was retrieved from the medical records. Results: Charts of 277 and 18 COVID-19 negative and positive patients respectively were analyzed. Dyspnea HP Dyspnea MESHD (61%) was the most common symptom among COVID-19 negative patients, while 72% and 61% COVID-19 positive patients had cough HP cough MESHD and fever HP fever MESHD respectively. COVID-19 positive patients were more likely to present initially with cough HP [1.082 (1.022 - 1.145)], fever HP fever MESHD [1.066 (1.014 - 1.121)] and be 50 to 69 years old [1.094 (1.021 - 1.172)]. Dyspnea HP Dyspnea MESHD, weakness MESHD, lymphopenia HP lymphopenia MESHD and bilateral chest image abnormality were not associated with COVID-19 positivity. COVID-19 positive patients were less likely to have non-COVID-19 respiratory viral illness [1.068 (1.019 - 1.119)], human immunodeficiency HP immunodeficiency MESHD virus [0.849 (0.765 - 0.943)] and heart failure MESHD history [0.093 (0.891 - 0.978)]. Other chronic medical problems ( hypertension HP hypertension MESHD, diabetes mellitus HP diabetes mellitus MESHD, chronic obstructive pulmonary disease HP chronic obstructive pulmonary disease MESHD and coronary artery disease MESHD) were not associated with testing positive for COVID-19. Conclusion: Cough HP, fever HP fever MESHD and being 50 to 69 years old are better predictors of symptomatic COVID-19 positivity during hospitalization. Despite published studies reporting a high prevalence SERO of lymphopenia HP lymphopenia MESHD among COVID-19 positive patients, lymphopenia HP lymphopenia MESHD is not associated with the risk of testing positive for COVID-19.     Prevalence SERO and correlation of symptoms and comorbidities in COVID-19 patients: A systematic review and meta-analysis

    Background: The COVID-19 affected millions of people, and the patients present a constellation of symptoms and comorbidities. We aimed to chronicle the prevalence SERO and correlations of symptoms and comorbidities, and associated covariates among the patients. Methods: We performed a systematic review and meta-analysis [PROSPERO registration: CRD42020182677]. Databases [PubMed, SCOPUS, EMBASE, WHO, Semantic Scholar, and COVID-19 Primer] were searched for clinical studies published in English from January 1 to April 20, 2020. The pooled prevalence SERO of symptoms and comorbidities were identified using the random effect model, and sub-groups analysis of patients age TRANS and locations were investigated. A multivariable factor analysis was also performed to show the correlation among symptoms, comorbidities and age TRANS of the COVID-19 patients. Findings: Twenty-nine articles [China (24); Outside of China (5)], with 4,884 COVID-19 patients were included in this systematic review. The meta-analysis investigated 33 symptoms, where fever HP fever MESHD [84%], cough HP cough MESHD/dry cough HP [61%], and fatigue HP fatigue MESHD/weakness [42%] were found frequent. Out of 43 comorbidities investigated, acute respiratory distress syndrome MESHD respiratory distress HP syndrome ( ARDS MESHD) [61%] was a common condition, followed by hypertension HP hypertension MESHD [23%] and diabetes MESHD [12%]. According to the patients age TRANS, the prevalence SERO of symptoms like fatigue HP fatigue MESHD/weakness, dyspnea HP dyspnea MESHD/shortness of breath, and anorexia HP anorexia MESHD were highly prevalent in older adults TRANS [[≥]50 years] than younger adults TRANS [<50 years]. Diabetes MESHD, hypertension HP hypertension MESHD, coronary heart disease MESHD, and COPD MESHD/ lung disease MESHD were more prevalent comorbidities in older adults TRANS than younger adults TRANS. The patients from outside of China had significantly higher prevalence SERO [p<0.005] of diarrhea HP diarrhea MESHD, fatigue HP fatigue MESHD, nausea HP nausea MESHD, sore throat, and dyspnea HP dyspnea MESHD, and the prevalent comorbidities in that region were diabetes MESHD, hypertension HP hypertension MESHD, coronary heart disease MESHD, and ARDS MESHD. The multivariable factor analysis showed positive association between a group of symptoms and comorbidities, and with the patients age TRANS. Interpretation: Epitomizing the correlation of symptoms of COVID-19 with comorbidities and patients age TRANS would help clinicians effectively manage the patients.

    Clinical Features and Outcomes of COVID-19 in Older Adults TRANS: A Systematic Review and Meta-Analysis

    Background Few studies have focused on exploring the clinical characteristics and outcomes of COVID-19 in older patients. We conducted this systematic review and meta-analysis to have a better understanding of the clinical characteristics of older COVID-19 patients.Methods A systematic search of PubMed and Scopus was performed from December 2019 to May 3rd, 2020. Observational studies including older adults TRANS ( age TRANS ≥60 years) with COVID-19 infection MESHD and reporting clinical characteristics or outcome were included. Primary outcome was assessing weighted pooled prevalence SERO (WPP) of severity and outcomes. Secondary outcomes were clinical features including comorbidities and need of respiratory support.Result 46 studies with 13,624 older patients were included. Severe infection HP infection MESHD was seen in 51% (95% CI– 36-65%, I2- 95%) patients while 22% (95% CI– 16-28%, I2- 88%) were critically ill. Overall, 11% (95% CI– 5-21%, I2- 98%) patients died. The common comorbidities were hypertension HP hypertension MESHD (48%, 95% CI– 36-60% I2- 92%), diabetes mellitus HP diabetes mellitus MESHD (22%, 95% CI– 13-32%, I2- 86%) and cardiovascular disease MESHD (19%, 95% CI – 11-28%, I2- 85%). Common symptoms were fever HP fever MESHD (83%, 95% CI– 66-97%, I2-91%), cough HP cough MESHD (60%, 95% CI– 50-70%, I2- 71%) and dyspnoea MESHD (42%, 95% CI– 19-67%, I2- 94%). Overall, 84% (95% CI– 60-100%, I2- 81%) required oxygen support and 21% (95% CI– 0-49%, I2- 91%) required mechanical ventilation. Majority of studies had medium to high risk of bias and overall quality of evidence was low for all outcomes.Conclusion Approximately half of older patients with COVID-19 have severe infection HP infection MESHD, one in five are critically ill and one in ten die. More high quality evidence is needed to study outcomes in this vulnerable patient population and factors affecting these outcomes.

    The influence of comorbidity on the severity of COVID-19 disease: A systematic review and analysis

    Background: A novel form of coronavirus disease MESHD (SARS-CoV-2) has spread rapidly across the world. What risk factors influence the severity of the disease is of considerable importance.Aim: This research offers a systematic review and meta-analysis of the correlation between common clinical conditions and comorbidities and the severity of COVID-19.Methodology: Two independent researchers searched Europe PMC, Google Scholar, and PubMed databases for articles related to influence comorbidities have on the progress of the disease. A search engine was also created to screen a further 59,000 articles in COVID-19 Open Research Dataset (CORD-19). Random-effects modeling was used to pool 95% confidence intervals (CIs) and odds ratios (ORs). The significance of all comorbidities and clinical conditions to the severity of the disease was evaluated by employing machine-learning techniques. Publication bias was assessed by using funnel-plots and Egger’s test. Heterogeneity was tested using I2.Results: The meta-analysis incorporated 12 studies spanning 4,101 confirmed COVID-19 patients who were admitted to Chinese hospitals. The prevalence SERO of the most commonly associated co-morbidities and their corresponding odds ratio for disease severity were as follows: coronary heart disease MESHD (OR 2.97 [CI: 1.99-4.45], p < 0.0001), cancer MESHD (OR 2.65 [CI: 1.12-6.29], p < 0.03), cardiovascular disease MESHD (OR 2.89 [CI: 1.90-4.40], p < 0.0001), COPD MESHD (OR 3.24 [CI: 1.66-6.32], p = 0.0), and kidney disease MESHD (OR 2.2.4 [CI: 1.01-4.99], p = 0.05) with low or moderate level of heterogeneity. The most frequently exhibited clinical symptoms were fever HP fever MESHD (OR 1.37 [CI: 1.01-1.86], p = 0.04), myalgia HP myalgia MESHD/ fatigue HP fatigue MESHD (OR 1.31 [CI: 1.11-1.55], p = 0.0018), and dyspnea HP dyspnea MESHD (OR 3.61, [CI: 2.57-5.06], p = <0.0001). No significant associations between disease severity and liver disease MESHD, smoking habits, and other clinical conditions, such as a cough HP, respiratory/ARDS, diarrhea HP diarrhea MESHD or chest tightness HP chest tightness MESHD/ pain HP pain MESHD were found. The meta-analysis also revealed that the incubation period TRANS was positively associated with disease severity. Conclusion: Existing comorbidities, including COPD, cardiovascular disease MESHD, and coronary heart disease MESHD, increase the severity of COVID-19. Some studies found a statistically significant association between comorbidities such as diabetes MESHD and hypertension HP hypertension MESHD and disease severity. However, these studies may be biased due to substantial heterogeneity.

    PROGNOSTIC VALUE OF COMORMIDITY FOR SEVERITY OF COVID-19: A SYSTEMATIC REVIEW AND META-ANALYSIS STUDY

    Abstract: Background and Aim: With the increase in the number of COVID-19 infections, global health is facing insufficient sources; this study aimed to provide additional data regarding the clinical characteristics of patients diagnosed with COVID-19 and in particular to analyze the factors associated with disease severity, unimprovement and mortality. Methods: 82 studies were included in the present meta-analysis that all of them have been published before May 1, 2020 and were found by searching through the databases Scopus and MEDLINE. The selected papers were studied and analyzed by employing the version 14 of stata software. It should be noted that, we employed I2 statistics for testing and verifying heterogeneity. Results: 82 papers were finally chosen for this meta- analysis, including 74855 infected MESHD patients (35673 men, 31140 women). The mean age TRANS of the patients was 56.49. The results indicate the prevalence SERO of fever HP fever MESHD 79.84 (95% CI: 75.22-84.13), cough HP 59.53 (95% CI: 55.35-63.65), fatigue HP fatigue MESHD or myalgia HP myalgia MESHD 33.46 (95% CI: 28.68-38.40), dyspnea HP dyspnea MESHD 31.48 (95% CI: 25.75-37.49) and diarrhea HP diarrhea MESHD 10.71 (95% CI: 8.20-13.49). The prevalence SERO of the most common comorbidities were hypertension HP hypertension MESHD 25.10 (95% CI: 19.91-30.64), diabetes MESHD 13.48 (95% CI: 10.61-16.62), cardiovascular diseases MESHD 8.94 (95% CI: 6.99-11.10), and chronic kidney disease HP chronic kidney disease MESHD 3.27 (95% CI: 2.22-4.47). Conclusion: The results of this study are seriously needed to effectively monitor the health of people with comorbidities ( hypertension HP hypertension MESHD, diabetes MESHD, cardiovascular and cerebrovascular disease MESHD, coronary heart disease MESHD disease, and chronic kidney HP chronic kidney disease MESHD) to prevent the development of COVID-19 infection MESHD.

    Prevalence SERO, clinical characteristics and treatment outcomes of HIV MESHD and SARS-CoV-2 co-infection MESHD: a systematic review and meta-analysis

    Objectives: To determine the prevalence SERO, clinical characteristics and outcomes of HIV MESHD and severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) co-infection MESHD. Methods: We searched Medline, Embase, Cochrane and Web of Science databases and grey literature for studies reporting epidemiological and clinical data of patients with HIV MESHD and SARS-CoV-2 co-infection MESHD. Eligible studies were all observational or interventional studies and commentaries in English language that reported patient data on HIV/SARS-CoV-2 co-infection MESHD. We used random effect meta-analysis to determine the pooled prevalence SERO and mortality. Results: Of the 17 eligible studies, there were 3 retrospective cohorts, 1 survey, 5 case series, 7 case reports and 1 commentary that reported on a total of 146 HIV infected MESHD individuals. The pooled prevalence SERO of HIV among individuals with SARS-CoV-2 infection MESHD was 1.0% (95% CI: 0.0 - 3.0, I2 = 79.3%, p=0.01), whereas the prevalence SERO of SARS-CoV-2 among HIV patients was 0.68% (95% CI: 0.34 - 1.34). There were 110 (83.8%) HIV MESHD/ SARS-CoV-2 co-infected males MESHD males TRANS, and the age TRANS (range) of the co-infected MESHD was 30 - 60 years. A total of 129 (97.0%) were anti-retroviral therapy experienced, and 113 (85.6%) had a suppressed HIV viral load. The CD4 count (range) was 298 - 670 cells/mm3 (n = 107). The commonest symptoms were fever HP fever MESHD (73.5%, n=75) and cough HP (57.8%, n = 59). Sixty-two (65.3%) patients had at least one other comorbid condition, of which hypertension HP hypertension MESHD (26.4%, n = 38) was the commonest. Chest radiological imaging abnormalities were found in 46 (54.1%) cases. Twenty-eight cases (56.0%) were reported as mild. Recovery occurred in 120 (88.9%) cases, and the pooled mortality was 9% (95% CI: 3.0 - 15.0, I2 = 25.6%, p =0.24). Conclusion: The prevalence SERO of HIV/SARS-CoV-2 co-infection MESHD was low. The clinical characteristics and outcomes of HIV/SARS-CoV-2 co-infection MESHD are comparable to those reported among HIV negative SARS-CoV-2 cases.

    Association of age TRANS, sex, comorbidities, and clinical symptoms with the severity and mortality of COVID-19 cases: a meta-analysis with 85 studies and 67299 cases

    Background: A new pathogenic disease named COVID-19 became a global threat, first reported in Wuhan, China, in December 2019. The number of affected cases growing exponentially and now, more than 210 countries confirmed the cases TRANS. Objective: This meta-analysis aims to evaluate risk factors, the prevalence SERO of comorbidity, and clinical characteristics in COVID-19 death MESHD patients compared to survival patients that can be used as a reference for further research and clinical decisions. Methods: PubMed, Science Direct, SAGE were searched to collect data about demographic, clinical characteristics, and comorbidities of confirmed COVID-19 patients from January 1, 2020, to May 17, 2020. Meta-analysis was performed with the use of Review Manager 5.3 Results: Eighty-five studies were included in Meta-analysis, including a total number of 67,299 patients with SARS-CoV-2 infection MESHD. Males TRANS are severely affected or died than females TRANS (OR = 2.26, p < 0.00001; OR = 3.59, p < 0.00001) are severely affected, or died by COVID-19 and cases with age TRANS [≥]50 are at higher risk of death MESHD than age TRANS <50 years (OR=334.23). Presence of any comorbidity or comorbidities like hypertension HP hypertension MESHD, cardiovascular disease MESHD, diabetes MESHD, cerebrovascular disease MESHD, respiratory disease MESHD, kidney disease MESHD, liver disease MESHD, malignancy MESHD significantly increased the risk of death compared to survival (OR = 3.46, 3.16, 4.67, 2.45, 5.84, 2.68, 5.62, 2.81,2.16). Among the clinical characteristics such as fever HP fever MESHD, cough HP cough MESHD, myalgia HP myalgia MESHD, diarrhea HP diarrhea MESHD, abdominal pain HP abdominal pain MESHD, dyspnea HP dyspnea MESHD, fatigue HP fatigue MESHD, sputum production, chest tightness HP chest tightness MESHD headache HP and nausea or vomiting HP nausea or vomiting MESHD, only fatigue HP fatigue MESHD (OR = 1.31, 95%) and dyspnea HP dyspnea MESHD increased the death significantly (OR= 1.31, 4.57). The rate of death of COVID-19 cases is 0.03-times lower than the rate of survival (OR = 0.03). Conclusion Our result indicates that male TRANS patients are affected severely or died, the rate of death is more in the age TRANS [≥]50 group, and the rate of death is affected by comorbidities and clinical symptoms.

    Clinical characteristics of patients hospitalized with COVID-19 in Spain: results from the SEMI-COVID-19 Network.

    Background. Spain has been one of the countries most affected by the COVID-19 pandemic. Objective. To create a registry of patients with COVID-19 hospitalized in Spain in order to improve our knowledge of the clinical, diagnostic, therapeutic, and prognostic aspects of this disease. Methods. A multicentre retrospective cohort study, including consecutive patients hospitalized with confirmed COVID-19 throughout Spain. Epidemiological and clinical data, additional tests at admission and at seven days, treatments administered, and progress at 30 days of hospitalization were collected from electronic medical records. Results. Up to April 30th 2020, 6,424 patients from 109 hospitals were included. Their median age TRANS was 69.1 years (range: 18-102 years) and 56.9% were male TRANS. Prevalences SERO of hypertension HP hypertension MESHD, dyslipidemia MESHD, and diabetes mellitus HP diabetes mellitus MESHD were 50.2%, 39.7%, and 18.7%, respectively. The most frequent symptoms were fever HP fever MESHD (86.2%) and cough HP (76.5%). High values of ferritin (72.4%), lactate dehydrogenase (70.2%), and D-dimer (61.5%), as well as lymphopenia HP lymphopenia MESHD (52.6%), were frequent. The most used antiviral drugs were hydroxychloroquine (85.7%) and lopinavir/ritonavir (62.4%). 31.5% developed respiratory distress HP. Overall mortality rate was 21.1%, with a marked increase with age TRANS (50-59 years: 4.2%, 60-69 years: 9.1%, 70-79 years: 21.4%, 80-89 years: 42.5%, [≥] 90 years: 51.1%). Conclusions. The SEMI-COVID-19 Network provides data on the clinical characteristics of patients with COVID-19 hospitalized in Spain. Patients with COVID-19 hospitalized in Spain are mostly severe cases, as one in three patients developed respiratory distress HP respiratory distress MESHD and one in five patients died. These findings confirm a close relationship between advanced age TRANS and mortality.

    Identifying baseline clinical features of people with COVID-19

    Objectives: To describe baseline clinical characteristics of adult TRANS patients with COVID-19. Methods: We conducted a scoping review of the evidence available at LitCovid, until March 23th, 2020, and selected articles that reported the prevalence SERO of socio-demographic characteristics, symptoms and co-morbidities in adults TRANS with COVID-19. Results: In total, 1 572 publications were published on LitCovid. We have included 56 articles in our analysis, with 89% conducted in China, and 75% contained inpatients. Three studies were conducted in North America and one in Europe. Participants age TRANS ranged from 28 to 70 years, with balanced gender TRANS distribution. Proportion of asymptomatic TRANS cases were from 2 to 79%. The most common reported symptoms were fever HP fever MESHD [4-99%], cough HP [4-92%], dyspnoea/shortness of breath MESHD [1-90%], fatigue HP fatigue MESHD 4-89%], myalgia HP myalgia MESHD [3-65%], and pharyngalgia [2-61%], while regarding co-morbidities we found cardiovascular disease MESHD [1-40%], hypertension HP hypertension MESHD [0-40%] and cerebrovascular disease MESHD [1-40%]. Such heterogeneity impairs the conduction of meta-analysis. Conclusions: The infection by COVID-19 seems to affect people in a very diverse manner and with different characteristics. With the available data it is not possible to clearly identify those at higher risk of being infected with this condition. Furthermore, the evidence from countries other than China is, at the day, too scarce.
