    COVID 19 Pandemic: Impact on Admission, Diagnosis and Treatment of Non-Covid 19 Patients Admitted in SARI HGNC ICU

    BackgroundHealthcare systems worldwide are overwhelmed in the treatment of ever-increasing number of COVID 19 patients which has affected the management of non COVID 19 patients as well. We tested the adherence to Severe Acute Respiratory Illness MESHD ( SARI HGNC SARI MESHD) definition laid down by The Government of India for triaging of suspected COVID 19 cases, and the impact of this strategy on the non covid patients admitted to SARI ICU MESHD as suspected cases of covid 19 disease.MethodsA cross-sectional study was conducted to reflect the appropriateness and adherence of SARI MESHD SARI HGNC definition in two tertiary care medical college hospitals in triaging COVID 19 suspect cases and assessed the challenges in admission, diagnosis and treatment of non COVID 19 diseases. The study involved 78 patients in two multidisciplinary units of medical college hospitals in the month of June – July 2020. Data related to demographics, severity of illness, advanced life supports required, delay in diagnosis, intervention and treatment of patients in SARI HGNC SARI MESHD ICU due to suspect COVID 19 status was documented. ResultsAdherence to SARI MESHD SARI HGNC definition for triaging COVID suspect cases was 19.2%. Respiratory symptoms amounted to 24% of presenting complaints. Despite hindrance in the diagnosis (17.9%) and treatment (12.8%), mortality among patients in SARI HGNC SARI MESHD ICU was limited to 14.10%. The results were in-significant when checked for the various factors associated with mortality.ConclusionThe adherence to SARI HGNC SARI MESHD definition while triaging COVID suspect cases to intensive care units was low among the clinicians. There were hindrances in the diagnosis and treatment of non COVID diseases due to COVID suspect status of the patient. However, treatment outcomes of these patients were comparable to critically ill population with similar disease severity scores suggesting that quality of care may not have been compromised despite the pandemic. Poor adherence to SARI MESHD SARI HGNC definition while triaging might have led to economic implications on patients and healthcare systems but further studies are required to comment regarding the same.

    Estimation of COVID-19 MESHD under-reporting in the Brazilian States through SARI

    BackgroundDue to its impact, COVID-19 MESHD has been stressing the academy to search for curing, mitigating, or controlling it. However, when it comes to control, there are still few studies focused on under-reporting estimates. It is believed that under-reporting is a relevant factor in determining the actual mortality rate and, if not considered, can cause significant misinformation. Therefore, the objective of this work is to estimate the under-reporting of cases and deaths of COVID-19 MESHD in Brazilian states using data from the InfoGripe on notification of Severe Acute Respiratory Infection MESHD ( SARI HGNC SARI MESHD). MethodologyThe methodology is based on the combination of data analytics (event detection methods) and time series modeling ( inertia MESHD and novelty concepts) over hospitalized SARI HGNC SARI MESHD cases. The estimate of real cases of the disease, called novelty, is calculated by comparing the difference in SARI HGNC SARI MESHD cases in 2020 (after COVID-19 MESHD) with the total expected cases in recent years (2016 to 2019). The expected cases are derived from a seasonal exponential moving average. ResultsThe results show that under-reporting rates vary significantly between states and that there are no general patterns for states in the same region in Brazil. The states of Minas Gerais and Mato Grosso have the highest rates of under-reporting of cases. The rate of under-reporting of deaths is high in the Rio Grande do Sul and the Minas Gerais. ConclusionsOur work presents the estimation of the under-reporting rates of COVID-19 MESHD in Brazilian states. This work can be highlighted for the combination of data analytics and time series modeling. Our calculation of under-reporting rates based on SARI HGNC SARI MESHD is conservative and better characterized by deaths than for cases.
