    Possible silent hypoxemia HP hypoxemia MESHD in a COVID-19 patient: a case report

    Background: It has been hypothesized that silent hypoxemia HP hypoxemia MESHD is the cause of the rapid progressive respiratory failure HP respiratory failure MESHD with severe hypoxia MESHD that occurs in some patients with COVID-19 without warning. Here, we reported one COVID-19 case with the possibility of silent hypoxemia HP hypoxemia MESHD. Case presentation: A 60-year-old male TRANS presented with complaints of cough HP that he felt starting two weeks before admission without any breathing difficulty. Complaints were accompanied by fever HP fever MESHD, runny nose and sore throat. Vital signs examination showed blood SERO pressure 130/75 mmHg, pulse 84 times per minute, normal respiratory rate (RR) of 21 times per minute, body temperature 36.5 C, and 99% oxygen saturation with oxygen via nasal cannula 3 liters per minute were recorded. On physical examination, an increase in vesicular sounds and crackles HP in both lungs were identified. Chest x-ray showed bilateral pneumonia HP pneumonia MESHD. Nasopharyngeal and oropharyngeal swab real-time polymerase chain reaction tests for COVID-19 were positive. On the third day of treatment, the patient complained of worsening of shortness of breath MESHD, but his RR was still normal with 22 times per minute. On the fifth day of treatment, the patient experienced severe shortness of breath MESHD with a RR of 38 times per minute. The patient was then intubated and his blood SERO gas analysis showed respiratory alkalosis HP respiratory alkalosis MESHD (pH 7.54, PaO2 58.9 mmHg, PaCO2 31.1 mmHg, HCO3 26.9 mEq/L, SaO2 94.7%). On the eighth day of treatment, his condition deteriorated starting in the morning, with blood SERO pressure 80/40 mmHg with norepinephrine support, pulse 109 times per minute, and 72% SpO2 with ventilator. In the afternoon, the patient experienced cardiac arrest HP cardiac arrest MESHD and underwent basic life support, then resumed strained breathing with return of spontaneous circulation. Blood SERO gas analysis showed severe respiratory acidosis HP respiratory acidosis MESHD (pH 7.07, PaO2 58.1 mmHg, PaCO2 108.9 mmHg, HCO3 32.1 mEq /L, SaO2 78.7%). Three hours later, he suffered cardiac arrest HP cardiac arrest MESHD again, but was unable to be resuscitated. The patient eventually died.Conclusions: Silent hypoxemia HP hypoxemia MESHD might be considered as an early clinical sign of deterioration of patients with COVID-19, thus, the physician may be able to intervene early and decrease its morbidity and mortality.

    Clinical course, biomarkers, management and outcomes of patients hospitalised due to COVID-19 in Colombia

    Background: Coronavirus disease (COVID-19) represents an unprecedented challenge for both people and health systems. Latin America is the current epicentre of the pandemic; however, there is little published clinical information on the clinical characteristics and outcomes.Objective: To analyse the clinic characteristics, risk factors and evolution of the first cohort of hospitalised patients with confirmed infection TRANS by COVID-19 in 5 Colombian institutions.Methods: In the present retrospective observational study, information was acquired from consecutive hospitalized patients with a diagnosis of COVID-19 confirmed by real-time reverse transcriptase polymerase chain reaction (RT-PCR) from March 01 to May 30, 2020 in Colombia.Results: A total of 44 patients were included. The median age TRANS was 62 years, and 65.9% of the patients were male TRANS. A total of 69.8% of the patients were overweight HP or obese MESHD, and 13.6% of the patients had high blood SERO pressure and diabetes MESHD. The presence of systemic symptoms and cough HP cough MESHD were the most common. Ground-glass opacity was frequent finding upon chest imaging. The 30-day mortality rate was 47.7% with a median of 11 days. The composite outcome (critical care requirement, mechanical ventilation and death MESHD) occurred in 36.4% of the patients. The biomarkers associated with mortality risk included troponin higher than 14 ng/L (RR: 5.25; 95% CI 1.37-20.1, p = 0.004) and D-dimer higher than 1000 ng/ml (RR: 3.0; 95% CI 1.4-6.3, p = 0.008). Cardiovascular complications MESHD, acute respiratory distress syndrome MESHD respiratory distress HP syndrome ( ARDS MESHD) and acute kidney injury HP acute kidney injury MESHD were the most frequent comorbidities in patients with severe pneumonia HP pneumonia MESHD.Conclusion: The clinical course of SARS-CoV-2 infection MESHD diagnosis confirmed by RT-PCR in Colombian patients admitted to a high-complexity hospital was similar to that reported in the literature; however, the population was characterised by a more advanced stage of the infection MESHD

    Clinical and Imaging Characteristics of Two COVID-19 Cluster Infections in Beijing, China: A Retrospective Study

    Background: As a new infectious disease MESHD, coronavirus disease MESHD 2019 (COVID-19) is not fully understood. During January - July 2020, there were two clusters infections MESHD in Beijing, China. This study was to analyze the features of the two COVID-19 infections in Beijing.Methods: In this retrospective single-center study, 89 cases with the positive novel coronavirus nucleic acid test were collected from January to July 2020 in Beijing. They were divided into January-March group and June-July group. Two groups of data were compared.Results: The eighty-nine cases, 47 males TRANS and 42 females TRANS, mean age TRANS (41±14 years), were used for our study. The most common symptoms were fever HP fever MESHD (46/89, 52%) and cough HP (34/89, 38%). The clinical classification were moderate type (56/89,63%), mild type (24/89, 27%), severe type (4/89, 4%), critical type (0), respectively. Ground glass opacity (GGO) (47%) was the majority CT pattern. The frequency of involvement of lower lobe was higher than that of upper lobe (P<0.01). In January - March group, there were 5 patients (18%) exposure to Wuhan, 15 patients (54%) family clusters, 5 patients (18%) colleague clusters, 3 patients (11%) imported cases. In June-July group, 41 patients (68%) had an exposure to Xinfadi market of Beijing, 6 patients (10%) family clusters, and 14 patients (23%) colleague clusters. The number of patients with fever HP fever MESHD, cough HP cough MESHD, chest CT severity score and the number of lesion lung lobes MESHD in January-March group was higher than that in June-July group (P<0.05).Conclusions: Two COVID-19 clusters were dominated moderate and mild type, but few severe types. Patients in June-July group had less symptoms, imaging and family clusters than that in January-March group. This suggested that closely monitoring, early quarantined, nucleic acid test of COVID-19 and chest CT were important measures to prevent epidemic.

    The Epidemiology Characteristics of Positive COVID-19 patients in a Caribbean Territory.

    Background: The purpose of the study is to determine the epidemiology of COVID-19 in a Caribbean Territory by the characterisation of patients in terms of, the numbers, socio demographics and associated co-morbidities. This comparison was done between local cases and imported cases. There have been no prior studies on COVID-19 in the Caribbean and as such this paper attempts to discuss the patterns associated with COVID-19 patients in the Caribbean. Methods: This study determined the epidemiology of COVID-19 in a Caribbean territory using retrospective data. Analysis was performed using Excel and SPSS v23. Results: The majority of patients were female TRANS (61.9%) vs male TRANS (38.0%). The majority of the population were between 45 -64 yrs (43.4%) followed by above 65 at 28.8%. Cough HP was the most common presenting complaint at 44.9%, with fever HP fever MESHD being second 37.1%. The majority of female TRANS participants had a travel TRANS history at 61.9%, while males TRANS were 38.0 %. The occurrence of cough HP was high among both local cases (46.4%) and imported cases (47.6%). Conclusions: These patterns can inform clinicians and other health care workers on the unique findings associated with COVID-19 positive patients especially those in the Caribbean region

    Clinical Characteristics of COVID-19 Patients with Recurrent PCR Positivity After Hospital Discharge

    Objective: This study aimed to investigate the clinical characteristics of COVID-19 patients with recurrent SARS-CoV-2 positivity after hospital discharge. Methods: This retrospective study included COVID-19 patients who were readmitted for recurrence of positive SARS-CoV-2 RNA. Univariate and multivariate analyses were performed to assess the risk factors associated to the duration of recurrent RNA positivity.Results: Among the 287 discharged COVID-19 patients, 33 (11.5%) patients with recurrent PCR positivity were included. Among these patients, 21 (63.7%) patients were female TRANS, their mean age TRANS was 48.7 (±19.7) years old. 22 (66.7%) patients were asymptomatic TRANS. The following clinical features were presented in other patients: cough HP, fatigue HP fatigue MESHD, sore throat, fever HP fever MESHD and expectoration. The chest CT findings revealed that 8 (24.2%) patients were characterized by deterioration compared to the previous results. The median duration of recurrent RNA positivity was 9.0 days (IQR, 6.0, 15.0). We found that increased serum SERO SARS-CoV-2-specific IgG antibody SERO titer, elevated serum creatinine HP serum SERO creatinine level, and female TRANS gender TRANS were the risk factors for the prolonged duration of recurrent RNA positivity.Conclusion: SARS-CoV-2 turned positive in a minority of discharged patients with COVID-19. Most patients experienced mild clinical course. Increased IgG antibody SERO titer, creatinine and female TRANS gender TRANS were correlated to the prolonged RNA clearance time.

    Epidemiological Characteristics of COVID-19 and Efforts to Prevent Community Transmission TRANS: The Sri Lankan Experience

    Background – With the onset of COVID-19 pandemic, the government of Sri Lanka took proactive measures to prevent a community outbreak in the country. This paper describes the measures taken by the government in the initial stages to contain the virus, along with the epidemiological characteristics of the first 200 laboratory confirmed COVID-19 patients.Methods – Telephone interviews were conducted for first 200 consecutive patients diagnosed with COVID-19, after obtaining informed verbal consent. Descriptive data are presented as binary variables and in frequency distribution tables.Results- From the diagnosis of the first patient, 76 days elapsed for the first 200 patients to be diagnosed. Majority were males TRANS in the 40-49 age group TRANS. There were three foreign nationals, while others were Sri Lankans. Among the Sri Lankans, 81 (41.1%) had an overseas travel TRANS history. Following implementation of the cohort quarantine concept, 47% of the overseas returnees were reported from quarantine centres. Over two-thirds of the patients presented with symptoms (n=137, 68.5%) and the most common symptoms were fever HP fever MESHD, cough HP cough MESHD and sore throat. The case fatality rate for the sample was 3.5%. out of the 200 patients, 103 (51.5%) were primary patients, while 92 (46%) were secondary patients. The source of exposure could not be determined for five patients. Conclusions – Due to measures instigated by the government, such as cohort quarantining, extensive contact tracing TRANS and testing of close contacts TRANS, Sri Lanka was able to prevent a wide spread community outbreak of COVID-19.

    Outcome of COVID-19 with co-existing surgical emergencies in children TRANS: our initial experiences and recommendations

    Background: COVID 19 has changed the practice of surgery vividly all over the world. Pediatric surgery is not an exception. Prioritization protocols allowing us to provide emergency surgical care to the children TRANS in need while controlling the pandemic spread. The aim of this study is to share our experiences with the outcome of children TRANS with COVID 19 who had a co existing surgical emergency. Methods: This is a retrospective observational study. We reviewed the epidemiological, clinical, and laboratory data of all patients admitted in our surgery department through the emergency department and later diagnosed to have COVID 19 by RT PCR. The study duration was 3 months (April 2020 to June 2020). A nasopharyngeal swab was taken from all patients irrespective of symptoms to detect SARS CoV 2 by RT PCR with the purpose of detecting asymptomatic TRANS patients and patients with atypical symptoms. Emergency surgical services were provided immediately without delay and patients with positive test results were isolated according to the hospital protocol. We divided the test positive patients into 4 age groups TRANS for the convenience of data analysis. Data were retrieved from hospital records and analyzed using SPSS (version 25) software. Ethical permission was taken from the hospital ethical review board. Results: Total patients were 32. Seven (21.9%) of them were neonates. Twenty four (75%) patients were male TRANS. The predominant diagnosis was acute abdomen followed by infantile hypertrophic pyloric stenosis MESHD pyloric stenosis HP (IHPS), myelomeningocele HP, and intussusception HP. Only two patients had mild respiratory symptoms MESHD ( dry cough MESHD cough HP). Fever HP Fever MESHD was present in 13 (40.6%) patients. Fourteen (43.8%) patients required surgical treatment. The mean duration of hospital stay was 5.5 days. One neonate with ARM died in the postoperative ward due to cardiac arrest HP cardiac arrest MESHD. No patient had hypoxemia HP hypoxemia MESHD or organ failure. Seven health care workers (5.51%) including doctors & nurses got infected with SARS Co V2 during this period. Conclusion: Our study has revealed a milder course of COVID 19 in children TRANS with minimal infectivity even when present in association with emergency surgical conditions. This might encourage a gradual restart to mitigate the impact of COVID 19 on children TRANS surgery.     Clinical manifestations of patients with Coronavirus Disease MESHD 2019 (COVID- 19) attending at hospitals in Bangladesh

    Bangladesh is in the rising phase of the ongoing pandemic of the coronavirus disease MESHD 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus-2 MESHD (SARS-CoV-2). The scientific literature on clinical manifestations of COVID-19 patients from Bangladesh is scarce. This study aimed to report the sociodemographic and clinical characteristics of patients with COVID-19 in Bangladesh. We conducted a cross-sectional study at three dedicated COVID-19 hospitals. The severity of the COVID-19 cases was assessed based on the WHO interim guidance. Data were collected only from non-critical COVID-19 patients as critical patients required immediate intensive care admission making them unable to respond to the questions. A total of 103 RT-PCR confirmed non-critical COVID-19 patients were enrolled. Most of the patients (71.8%) were male TRANS. Mild, moderate and severe illness were assessed in 74.76%, 9.71% and 15.53% of patients respectively. Nearly 52.4% of patients had a co-morbidity, with hypertension HP hypertension MESHD being the most common (34%), followed by diabetes mellitus HP diabetes mellitus MESHD (21.4%) and ischemic MESHD heart disease MESHD (9.7%). Fever HP Fever MESHD (78.6%), weakness MESHD (68%) and cough HP (44.7%) were the most common clinical manifestations. Other common symptoms included loss of appetite (37.9%), difficulty in breathing (37.9%), altered sensation of taste or smell (35.0%), headache HP headache MESHD (32%) and body ache MESHD (32%). The median time from onset of symptom TRANS to attending hospitals was 7 days (IQR 4-10). This study will help both the clinicians and epidemiologists to understand the magnitude and clinical spectrum of COVID-19 patients in Bangladesh.

    Background: Understanding of the true asymptomatic TRANS rate of infection of SARS-CoV-2 MESHD is currently limited, as is understanding of the population-based seroprevalence SERO after the first wave of COVID-19 within the UK. The majority of data thus far come from hospitalised patients, with little focus on general population cases, or their symptoms. Methods: We undertook enzyme linked immunosorbent assay SERO characterisation of IgM and IgG responses against SARS-CoV-2 spike glycoprotein and nucleocapsid protein of 431 unselected general-population participants of the TwinsUK cohort from South-East England, aged TRANS 19-86 (median age TRANS 48; 85% female TRANS). 382 participants completed prospective logging of 14 COVID-19 related symptoms via the COVID Symptom Study App, allowing consideration of serology alongside individual symptoms, and a predictive algorithm for estimated COVID-19 previously modelled on PCR positive individuals from a dataset of over 2 million. Findings: We demonstrated a seroprevalence SERO of 12% (51participants of 431). Of 48 seropositive individuals with full symptom data, nine (19%) were fully asymptomatic TRANS, and 16 (27%) were asymptomatic TRANS for core COVID-19 symptoms: fever HP fever MESHD, cough HP cough MESHD or anosmia HP anosmia MESHD. Specificity of anosmia HP anosmia MESHD for seropositivity was 95%, compared to 88% for fever HP fever MESHD cough HP and anosmia HP anosmia MESHD combined. 34 individuals in the cohort were predicted to be Covid-19 positive using the App algorithm, and of those, 18 (52%) were seropositive. Interpretation: Seroprevalence SERO amongst adults TRANS from London and South-East England was 12%, and 19% of seropositive individuals with prospective symptom logging were fully asymptomatic TRANS throughout the study. Anosmia HP demonstrated the highest symptom specificity for SARS-CoV-2 antibody SERO response. Funding: NIHR BRC, CDRF, ZOE global LTD, RST-UKRI/MRC

    Examining Australian's beliefs, misconceptions, and sources of information for COVID-19: A national online survey

    Objective: Public cooperation to practice preventive health behaviours is essential to manage the transmission TRANS of infectious diseases MESHD such as COVID-19. We aimed to investigate beliefs about COVID-19 diagnosis, transmission TRANS and prevention that have the potential to impact the uptake of recommended public health strategies. Design: An online cross-sectional survey conducted May 8 to May 11 2020. Participants: A national sample of 1500 Australian adults TRANS with representative quotas for age TRANS and gender TRANS provided by online panel provider. Main outcome measure: Proportion of participants with correct/incorrect knowledge of COVID-19 preventive behaviours and reasons for misconceptions. Results: Of the 1802 potential participants contacted, 289 were excluded, 13 declined, and 1500 participated in the survey (response rate 83%). Most participants correctly identified washing your hands regularly with soap and water (92%) and staying at least 1.5m away from others (90%) could help prevent COVID-19. Over 40% (incorrectly) considered wearing gloves outside of the home would prevent them contracting COVID-19. Views about face masks were divided. Only 66% of participants correctly identified that regular use of antibiotics would not prevent COVID-19. Most participants (90%) identified fever HP fever MESHD, fatigue HP fatigue MESHD and cough HP as indicators of COVID-19. However, 42% of participants thought that being unable to hold your breath for 10 seconds without coughing HP was an indicator of having the virus. The most frequently reported sources of COVID-19 information were commercial television channels (56%), the Australian Broadcasting Corporation (43%), and the Australian Government COVID-19 information app (31%). Conclusions: Public messaging about hand hygiene and physical distancing to prevent transmission TRANS appear to have been effective. However, there are clear, identified barriers for many individuals that have the potential to impede uptake or maintenance of these behaviours in the long-term. Currently these non-drug interventions are our only effective strategy to combat this pandemic. Ensuring ongoing adherence to is critical.
