    Older adults TRANS hospitalized with Covid-19: Clinical characteristics and early outcomes from a single center in Istanbul, Turkey

    Objective: Older adults TRANS have been continuously reported to be at higher risk for adverse outcomes of Covid-19. We aimed to describe clinical characteristics and early outcomes of the older Covid-19 patients hospitalized in our center comparatively with the younger patients, and also to analyze the triage factors that were related to the in-hospital mortality of older adults TRANS.Design: Retrospective; observational studySetting: Istanbul Faculty of Medicine hospital, TurkeyParticipants: 362 hospitalized patients with laboratory-confirmed Covid-19 from March 11 to May 11, 2020.Measurements: The demographic information; associated comorbidities; presenting clinical, laboratory, radiological characteristics on admission and outcomes from the electronic medical records were analyzed comparatively between the younger (<65 years) and older (≥65 years) adults TRANS. Factors associated with in-hospital mortality of the older adults TRANS were analyzed by multivariate regression analyses.Results: The median age TRANS was 56 years (interquartile range [IQR], 46-67), and 224 (61.9%) were male TRANS. There were 104 (28.7%) patients ≥65 years of age TRANS. More than half of the patients (58%) had one or more chronic comorbidity. The three most common presenting symptoms in the older patients were fatigue MESHD fatigue HP/ myalgia MESHD myalgia HP (89.4%), dry cough MESHD cough HP (72.1%), and fever MESHD fever HP (63.5%). Cough MESHD Cough HP and fever MESHD fever HP were significantly less prevalent in older adults TRANS compared to younger patients (p=0.001 and 0.008, respectively). Clinically severe pneumonia MESHD pneumonia HP was present in 31.5% of the study population being more common in older adults TRANS (49% vs. 24.4%) (p<0.001). The laboratory parameters that were significantly different between the older and younger adults TRANS were as follows: the older patients had significantly higher CRP, D-dimer, TnT, pro-BNP, procalcitonin levels, higher prevalence SERO of lymphopenia MESHD lymphopenia HP, neutrophilia HP, increased creatinine, and lower hemoglobin, ALT, albumin level (p<0.05). In the radiological evaluation, more than half of the patients (54.6%) had moderate-severe pneumonia MESHD pneumonia HP, which was more prevalent in older patients (66% vs. 50%) (p=0.006). The adverse outcomes were significantly more prevalent in older adults TRANS compared to the younger patients (ICU admission, 28.8% vs. 8.9%; mortality, 23.1% vs. 4.3%, p<0.001).  Among the triage evaluation parameters, the only factor associated with higher mortality was the presence of clinically severe pneumonia MESHD pneumonia HP on admission (Odds Ratio=12.3, 95% confidence interval=2.7-55.5, p=0.001).Conclusion: Older patients presented with more prevalent chronic comorbidities, less prevalent symptomatology but more severe respiratory signs and laboratory abnormalities than the younger patients. Among the triage assessment factors, the clinical evaluation of pulmonary involvement came in front to help clinicians to stratify the patients for mortality risk.

    COVID-19 infection MESHD among healthcare workers: a cross-sectional study in southwest Iran

    Objective: With the novel coronavirus pandemic, the impact on the healthcare system and workers cannot be overlooked.  However, studies on the infection MESHD status of medical personnel are still lacking. It is imperative to ensure the safety of health-care workers (HCWs) not only to safeguard continuous patient care but also to ensure they do not transmit the virus, therefore evaluation infection MESHD rates in these groups are indicated.Methods: Demographic and clinical data regarding infected cases among HCWs of Fars, Iran with positive SARS‐CoV‐2 PCR tests were obtained from 10th March to 16th May.Results: Our data demonstrated a rate of 5.62% (273 out of 4854 cases) infection MESHD among HCW, with a mean age TRANS of 35 years and a dominance of female TRANS cases (146 cases: 53.5%). The majority of infected cases were among nurses (51.3%) while the most case infection MESHD rate (CIR) was among physicians (27 out of 842 performed test (3.2%)). Also, the highest rate of infection MESHD was in the emergency MESHD rooms (30.6%). Also, 35.5% of the patients were asymptomatic TRANS and the most frequent clinical features among symptomatic patients were myalgia MESHD myalgia HP (46%) and cough MESHD cough HP (45.5%). Although 5.5% were admitted to hospitals, there were no reports of ICU admission. Furthermore, 10.3% of the cases reported transmitting the infection MESHD to family and friends TRANS. Regarding safety precautions, 1.6% didn't wear masks and 18.7% didn't use gloves in work environments. Conclusion: HCWs are among the highest groups at risk of infection TRANS risk of infection TRANS infection MESHD during the COVID-19 pandemic; therefore, analysis of the infection MESHD status of these groups is vital to maintain enough attention from the public, provide effective suggestions for government agencies and expanding protective measures is essential to decrease infection MESHD rates.

    A nomogram for predicting the severity of COVID-19 using laboratory examination and CT findings

    Background: The outbreak of COVID-19 has a significant impact on the health of people around the world. In the clinical condition of COVID-19, the condition of critical cases changes rapidly with a high mortality rate. Therefore, early prediction of disease MESHD severity and active intervention play an important role in the prognosis of severe patients.Methods: All the patients with COVID-19 in Taizhou city were retrospectively included and segregated into the non-severe and severe group according to the severity of the disease MESHD. The clinical manifestations, laboratory examination results, and imaging findings of the 2 groups were analysed for comparing the differences between the 2 groups. Univariate and multivariate logistic regression were used for screening the factors that could predict the disease MESHD, and the nomogram was constructed.Results: A total of 143 laboratory- confirmed cases TRANS were included in the study, including 110 non-severe patients and 33 severe patients. The median age TRANS of patients was 47 years (range, 4–86 years). Fever MESHD Fever HP (73.4%) and cough MESHD cough HP (63.6%) were the most common initial clinical symptoms. By using the method of multivariate logistic regression, the variables to construct nomogram include age TRANS (OR: 1.052, 95% CI: 1.020–1.086, P = 0.001), body temperature (OR: 2.252, 95% CI: 1.139–4.450, P = 0.020), lymphocyte count (OR: 1.128, 95% CI: 1.000–1.272, P = 0.049), ADA (OR: 1.163, 95% CI: 1.023–1.323, P = 0.021), PaO2 (OR: 0.972, 95% CI: 0.953–0.992, P = 0.007), IL-10 (OR: 1.184, 95% CI: 1.037–1.351, P = 0.012), and bronchiectasis MESHD bronchiectasis HP (OR: 3.818, 95% CI: 1.694–8.605, P = 0.001). The AUC of the established nomogram was 0.877.Conclusions: This study established a stable nomogram for predicting the severity of COVID-19, and the clinicians can use the established nomogram for predicting the severity of newly diagnosed COVID-19 patients and to conduct active intervention for minimising the mortality rate and improving the prognosis of severe patients.

    COVID-19 Case Series at UnityPoint Health St. Lukes Hospital in Cedar Rapids, IA

    A retrospective, descriptive study of all patients tested for SARS-CoV2 on admission for illness to a community hospital in Iowa from 3/21/20 thru 6/14/20 consisted of evaluation as to demographics, presentation and hospital course. Ninety-one patients were SARS-CoV2 PCR+ with 63% being male TRANS and a median age TRANS of 60. Cardiovascular disease MESHD was a significant comorbidity in the PCR+ group. Fever MESHD Fever HP, cough MESHD cough HP, dyspnea MESHD dyspnea HP, nausea MESHD nausea HP, emesis, diarrhea MESHD diarrhea HP, headache MESHD headache HP and myalgias MESHD myalgias HP were significantly more common in that group, as was an elevated CRP, LDH, serum SERO ferritin and transaminases. Overall survival of the COVID-19 patients was 88%, 77% in the critically ill, 59% of those mechanically ventilated and 33% of those requiring new dialysis. Survival was 93% in those not receiving any antivirals. Survival of those treated with hydroxychloroquine-azithromycin was 92%, compared to 86% of those treated with hydroxychloroquine alone. The latter two groups were significantly more ill than the untreated group. A transition from an early intubation strategy to aggressive utilization of high flow nasal cannula and noninvasive ventilation(i.e BiPAP) was successful in freeing up ICU resources.

    Probability of aerosol transmission TRANS of SARS-CoV-2

    Transmission TRANS of SARS-CoV-2 leading to COVID-19 occurs through exhaled respiratory droplets from infected humans. Currently, however, there is much controversy over whether respiratory aerosol microdroplets play an important role as a route of transmission TRANS. By measuring and modeling the dynamics of exhaled respiratory droplets we can assess the relative contribution of aerosols in the spreading of SARS-CoV-2. We measure size distribution, total numbers and volumes of respiratory droplets, including aerosols, by speaking and coughing MESHD coughing HP from healthy subjects. Dynamic modelling of exhaled respiratory droplets allows to account for aerosol persistence times in confined public spaces. The probability of infection MESHD by inhalation of aerosols when breathing in the same space can then be estimated using current estimates of viral load and infectivity of SARS-CoV-2. In line with the current known reproduction numbers TRANS, our study of transmission TRANS of SARS-CoV-2 suggests that aerosol transmission TRANS is an inefficient route, in particular from non or mildly symptomatic individuals.

    COVID-19 Detection on Chest X-Ray and CT Scan Images Using Multi-image Augmented Deep Learning Model

    COVID-19 is posed as very infectious and deadly pneumonia MESHD pneumonia HP type disease MESHD until recent time. Novel coronavirus or SARS-COV-2 strain is responsible for COVID-19 and it has already shown the deadly nature of respiratory disease MESHD by threatening the health of millions of lives across the globe. Clinical study reveals that a COVID-19 infected person may experience dry cough MESHD cough HP, muscle pain MESHD pain HP, headache MESHD headache HP, fever MESHD fever HP, sore throat and mild to moderate respiratory illness. At the same time, it affects the lungs badly with virus infection MESHD. So, the lung can be a prominent internal organ to diagnose the gravity of COVID-19 infection MESHD using X-Ray and CT scan images of chest. Despite having lengthy testing time, RT-PCR is a proven testing methodology to detect coronavirus infection MESHD. Sometimes, it might give more false positive and false negative results than the desired rates. Therefore, to assist the traditional RT-PCR methodology for accurate clinical diagnosis, COVID-19 screening can be adopted with X-Ray and CT scan images of lung of an individual. This image based diagnosis will bring radical change in detecting coronavirus infection MESHD in human body with ease and having zero or near to zero false positives and false negatives rates. This paper reports a convolutional neural network (CNN) based multi-image augmentation technique for detecting COVID-19 in chest X-Ray and chest CT scan images of coronavirus suspected individuals. Multi-image augmentation makes use of discontinuity information obtained in the filtered images for increasing the number of effective examples for training the CNN model. With this approach, the proposed model exhibits higher classification accuracy around 95.38% and 98.97% for CT scan and X-Ray images respectively. CT scan images with multi-image augmentation achieves sensitivity SERO of 94.78% and specificity of 95.98%, whereas X-Ray images with multi-image augmentation achieves sensitivity SERO of 99.07% and specificity of 98.88%. Evaluation has been done on publicly available databases containing both chest X-Ray and CT scan images and the experimental results are also compared with ResNet-50 and VGG-16 models.

    Analysis of Clinical Imaging Characteristics of Patients with 2019-nCoV

    Background: This article retrospectively analyzed the clinical characteristics and CT characteristics of chest CT of 10 patients with coronavirus disease MESHD 2019 (COVID-19) diagnosed in Heilongjiang Province. Methods: A retrospective analysis of 10 COVID-19 patients confirmed by designated hospitals in Heilongjiang Province. Results: The clinical manifestations were mainly fever MESHD fever HP (80%) and cough MESHD cough HP (50%). Laboratory tests showed elevated C-reactive protein (70%) and serum SERO amyloid A (80%). CT mainly manifests as ground-glass opacity (100%) in the lung, more than two lung lobes (90%), and the lesions are mostly located in the right upper lobe (80%), and lobular septum thickening (60%) is common.Conclusions: In this group of patients with COVID-19, the typical CT manifestations are double lung sheet ground-glass opacity, which are mainly distributed in the subpleural area, and the range of involvement is more than or equal to two lung lobes, “paving stones” changes; pulmonary lesions were absorbed and fibrous cord foci were formed during the transition period. It is worth noting that hilum, mediastinal lymphadenopathy HP lymphadenopathy MESHD, and pleural effusion MESHD pleural effusion HP are less common. During the study period, follow-up imaging performed by some patients showed mild / moderate disease progression MESHD and gradually dissipating.

    Clinical Characteristics of 107 Patients with COVID-19 in Ningbo, China: Single Center Experience Outside Hubei

    Background Since December 2019, there has be an outbreak of coronavirus disease MESHD 2019 (COVID-19) caused by severe acute respiratory syndrome MESHD coronavirus 2 (SARS-CoV-2) in Wuhan, China. Nowadays, it rapidly spread across the country and then the worldwide. We aimed to investigate the clinical characteristics of patients with COVID-19.Methods The patients with confirmed COVID-19 admitted between January 25 and February 10, 2020, were enrolled. Epidemiological, demographic, clinical, laboratory, radiological data, and antivirus therapies, were retrospectively collected and analyzed. The 90-day follow-up of these patients was also performed.Results A total of 107 patients were included. The median age TRANS was 55.0 years (range from 18.0 to 85.0 years), and 72 (67.3%) were female TRANS. Ninety-three (86.9%) of the patients had a history of contacting with residents from Wuhan (n=31), or contacting with confirmed COVID-19 patients (n=62) within 2 weeks. Fifty-eight (54.2%) had a family cluster onset. Fever MESHD Fever HP and cough MESHD cough HP were the most common symptoms. Only two patients had diarrhea MESHD diarrhea HP. The most common underlying disease MESHD was hypertension MESHD hypertension HP. Lymphopenia MESHD Lymphopenia HP was observed in 26 patients. Fifty-two patients with an elevated level of IL-6. On admission, bilateral patchy shadowing and ground-glass opacity were the typical radiological findings on chest computed tomography. Six patients had an intensive care unit (ICU) stay. Antivirus therapy was performed to all patients. 105 patients discharged with an improved condition, and no death MESHD was occurred during our 90-day follow-up for these patients.Conclusions Patients with COVID-19 in our hospital had relatively mild symptoms, and good prognosis. This study also highlights the importance of human-to-human transmission TRANS in COVID-19.

    Impact of Meteorological factors and population size on the transmission TRANS of Micro-size respiratory droplets based Coronavirus: A brief study of highly infected cities in Pakistan

    Ongoing Coronavirus epidemic (COVID-19) identified first in Wuhan, China posed huge impact on public health and economy around the globe. Both cough MESHD cough HP and sneeze MESHD sneeze HP based droplets or aerosols encapsulated COVID-19 particles are responsible for air borne transmission TRANS of this virus and caused unexpected escalation and high mortality worldwide. Current study intends to investigate correlation of COVID-19 epidemic with meteorological parameters particularly, temperature, rainfall, humidity, and wind speed along with population size. Data set of COVID-19 for highly infected cities of Pakistan was collected from the official website of National Institute of health (NIH). Spearman rank (rs) correlation coefficient test employed for data analysis revealed significant correlation between temperature minimum (TM), temperature average (TA), wind speed (WS) and population size (PS) with COVID-19 pandemic. Furthermore, receiver operating characteristics (ROC) curve was used to analyze the sensitivity SERO of TA, WS, and PS on transmission TRANS rate of COVID-19 in selected cities of Pakistan. The results obtained for sensitivity SERO and specificity analysis for all selected parameters signifies sensitivity SERO and direct correlation of COVID-19 transmission TRANS with temperature variation, WS and PS. Positive correlation and strong association of PS parameter with COVID-19 pandemic suggested need of more strict actions and control measures for highly populated cities. These findings will be helpful for health regulatory authorities and policymakers to take specific measures to combat COVID-19 epidemic in Pakistan.

    Anesthesia for an emergency MESHD gastrorrhagia operation in a patient suspected with COVID-19: a case report

    BackgroundThe spread of the Coronavirus Disease MESHD 2019 (COVID-19) in China since December 2019 was very grave. According to Chinese govenment's guidelines, patients can be divided into confirmed cases TRANS and suspected cases. The protective measures for both are the same.Case summaryWe report a case of an emergency MESHD gastrorrhagia operation for a patient suspected to have contracted COVID-19. A 54-year-old man with massive gastric hemorrhage MESHD was scheduled for an emergency MESHD operation for exploratory hemostasis. COVID-19 infection MESHD, however, could not be ruled out, and the patient was treated as a suspected case. All the protective measures were based on patients infected or suspected to be infected with COVID-19. Before the operation, we carried out a series of protective measures, such as preparing the operating room and wearing personal protective gear. We increased the dose of neuromuscular blocking agents during the induction of anesthesia to prevent the splashing of secretions by coughing MESHD coughing HP. The operation went smoothly, and the patient was transferred to an intensive care unit (ICU) in the isolation ward for extubation. Seven days later, the patient was ruled out for COVID-19 infection MESHD. ConclusionThe patient recovered successfully from the massive gastric hemorrhage MESHD, and the medical staff were not infected.
